POSITION SUMMARY RN Case Manager POSITION DUTIES UR; ECP MINIMUM REQUIRED QUALIFICATIONS Registered Nurse, Case Manager Case Management Experience required Outpatient or Inpatient Experience required Discharge Planning Experience required Patient Care Coordination experience PREFERRED QUALIFICATIONS Medicare and MediCal benefit knowledge LENGTH OF ASSIGNMENT 13 weeks SHIFT / HOURS PER WEEK 8D; 8a - 4:30p; M - F; Weekends/Call: Every 7th weekend SYSTEMS Epic/Health Connect experience START DATE ASAP/ 3/9/26
