POSITION SUMMARY – RN Case Manager POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization MINIMUM REQUIRED QUALIFICATIONS – CA RN license; AHA BLS; 2+ years recent RN Case Management experience, with experience in a LTAC or STACH setting, MCG, discharge planning across acute care settings, strong IDT collaboration LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – M-F 8a-430p. Weekends on request. START DATE – ASAP
