We are seeking a full-time, Case Manager for our Lowell Day Program. What does a Case Manager do? • Responsible for maintaining Guest Confidential Files in compliance with established requirements. • Responsible for writing grammatically sound ISP and DHSP goals, objectives and implementation strategies that focus on skill attainment. Writes comprehensive and grammatically sound progress notes and assessments in accordance with the timing requirements set forth by funding sources. • Responsible for developing an in-depth understanding of Guests strengths and needs and for communicating these strengths and needs in a professional manner to families/guardians, funding sources and fellow Colleagues. SPECIFIC DUTIES: • Works with Colleagues, Clinicians and the ISP/IDT team to develop ISPs/ DHSPs for each of the Guests. • Maintains a regular caseload and ensures all documentation is completed in a timely manner including assessments, datasheets, monthly, quarterly, semi-annual and annual reports as required by the applicable funding source. • Requests documentation from medical providers, residential providers and families/guardians as needed to ensure required documentation is maintained in the c-files. • Attends all ISP, IDT and ABI meetings and actively participates and provides leadership in those meetings. • Serves as liaison to families and residential providers around special scheduling needs, e.g., for doctors appointments, money needs, e.g., for special events and other special issues. Accompanies Guest(s) to hospitals in the event of a medical emergency. • Works in conjunction with the Program Directors and Program Coordinators to support the Guest referral process, including but not limited to the collection of medical records, completion of the Day Hab Leveling Tool and PT-1 Forms. • Keeps Case Management area clean to ensure a healthy, clean environment for the Guests. • Works with Colleagues to support Guest safety such as during fire drills and behavioral or medical emergencies. • Works with Colleagues to promote the understanding of goals, objectives, and datasheets; supports Colleagues to record quality data and monthly reporting. This may include training Colleagues in staff meetings. • Regularly observes and meets with Guests to gain firsthand knowledge of performance towards goals and objectives and the overall well-being of the Guests. • Meets regularly with Case Management Supervisor for continued professional development and growth. • Participates in various groups/committees and special projects, including but not limited to WOW! events and activities. • Other duties and responsibilities as assigned. SKILLS NEEDED/ESSENTIAL JOB FUNCTIONS: • Ability to maintain civility, poise, respect and professionalism at all times with Guests and their families/guardians, funding sources and Colleagues. Uses the highest standard of ethical conduct at all times and positively promotes the image of those with developmental disabilities and American Training, Inc. • Maintains knowledge and practice of Human Rights in all duties • Maintains confidentiality • Exceptionally strong communication, writing and relationship building skills required • Proper professional dress and demeanor • Attention to detail • Strong computer skills Requirements: • At least one year of experience in the Developmental Disabilities field. • If no Degree, must have at least 2 years of experience in the field of Developmental Disabilities • A valid drivers license and good driving record.
