Must have a current multistate RN license, 3+ years hospice or 3+ years home health experience with 1-2 years leadership, and strong clinical and operational leadership skills.
Overview Hospice and Home Health Leadership Required RN multistate license Required Competitive Pay: $100,000-120,000 Annual Salary Based on Experience Responsible for providing strategic and operational leadership to a team of Clinical Care Navigators who support home health care centers in optimizing patient transitions across the continuum of care. Responsibilities Directly supervises Clinical Care Navigators, providing guidance, coaching, and performance feedback. Serves as a clinical resource, mentor, and liaison, driving performance, quality, and consistency in care navigation practices. Oversees daily operations, ensuring timely and accurate execution of navigation activities. Evaluates and strategically adjusts caseloads for both personal assignments and Clinical Care Navigators to ensure balanced workloads, adequate coverage, and continuity of care. Applies clinical judgment and operational insight to prioritize needs based on acuity, staffing availability, and service demands. Ensures Clinical Care Navigators are effectively identifying patients at elevated risk, facilitating appropriate referrals, and collaborating with internal and external stakeholders to improve outcomes. Ensures appropriate use of risk stratification tools and clinical documentation review to identify high-risk patients. Supports Navigators in interpreting complex clinical indicators and making informed decisions about care transitions. Serves as an escalation point for complex cases requiring advanced clinical judgment. Coordinates with care center leadership, clinical teams, and referring providers to align navigation efforts with organizational goals. Leads the implementation of processes to ensure patient safety during transitions of care fostering multidisciplinary team collaboration across care settings. Acts as a strategic liaison across internal departments and external partners to optimize care coordination efforts. Reviews patient tracking and activity reports to identify trends, gaps, and opportunities for improvement. Contributes to the development and refinement of care navigation protocols, workflows, and metrics. Analyzes data to inform strategic decisions and enhance program effectiveness. Collaborates with leadership to align navigation services with broader clinical and operational strategies. Performs other duties as assigned. Qualifications Required- Current, unencumbered multistate license to practice as a Registered Nurse. Nurse License Compact (NLC) documents the following: The mutual recognition model of nurse licensure allows a nurse to have one license (in his or her state of residency) and to practice in other states (both physical and electronic), subject to each state's practice law and regulation. Nurses must legally reside in an NLC state to be eligible to have a multi-state license. See list of Nurse Licensure Compact States (NLC). Three (3+) years in hospice and one (1+) year prior leadership or supervisory experience. (May consider candidates with a minimum of 3 years home health relatable experience and two-years leadership or supervisory experience.)
