Job Title: Social Services Director Facility: Ambassador Healthcare at College Park Schedule: Full-Time | 8:00 AM – 4:30 PM Pay Range: $26.92 – $32.00 per hour About the Role Ambassador Healthcare at College Park is seeking a compassionate and dedicated Social Services Director to join our team. In this vital role, you’ll help ensure our residents receive the emotional, social, and community support they need to maintain the highest quality of life. You’ll work closely with residents, families, and the interdisciplinary team to create meaningful connections and resources that support overall well-being. Key Responsibilities • Implement the facility’s social services programs under the direction of the Administrator and Director of Social Services. • Participate in facility and individual resident assessments to determine social service needs. • Coordinate with community agencies to obtain resources that support residents’ needs. • Collaborate with the interdisciplinary team (IDT) to plan and deliver comprehensive resident care. • Assist residents and families with discharge planning, financial assistance, and community referrals. • Ensure residents receive necessary behavioral health services to maintain physical, mental, and psychosocial well-being. • Participate in resident and family council meetings as needed. • Educate residents on their rights, living wills, and related topics. • Stay current on federal and state regulations and recommend updates to facility policies. • Provide support and translation resources for residents with limited English proficiency. • Maintain a resource library of laws, standards, and best practices related to social services. • Participate in facility surveys and inspections as required. Qualifications • Bachelor’s Degree in Social Work or a Human Services field required. • Minimum of 1 year of experience in social work; long-term care experience preferred. • Strong working knowledge of PointClickCare or similar healthcare documentation systems. • Excellent interpersonal, communication, and organizational skills.
