Full job description Responsibilities: - Provide personal care and assistance to patients in their homes - Assist with activities of daily living, including bathing, dressing, and grooming - Medications reminder as directed by healthcare professionals - Monitor vital signs and report any changes or concerns to the appropriate medical personnel - Assist with mobility and transfers, using proper techniques and equipment - Help with light housekeeping tasks, such as meal preparation and laundry - Provide companionship and emotional support to patients - Follow care plans and maintain accurate documentation of services provided Qualifications: - Valid certification as a Home Health Aide (HHA) - Previous experience working is preferred - Knowledge of basic medical procedures, such as catheter care, colostomy care. - Familiarity with medical documentation - Understanding of behavioral health principles and techniques - Ability to lift and move patients safely - Excellent communication skills and the ability to work well in a team Note: This job description is not intended to be all-inclusive.
