Case Manager Nurse – Alliance Wound Care Services At Alliance Wound Care Services, we are committed to raising the standard of wound care by putting patients first. We are seeking a Case Manager LVN to join our team and play a vital role in coordinating patient care, supporting families, and ensuring providers have the resources they need to deliver the highest level of clinical care. This is a full-time, Remote position. Key Responsibilities • Coordinate patient care by managing schedules, records, and provider orders through our electronic systems. • Review and audit medical records to ensure completeness, accuracy, and timely follow-up on action items. • Communicate with patients, families, and providers to offer education, guidance, and advocacy. • Monitor new patient referrals and process intake information accurately and efficiently. • Support clinical staff by maintaining organized workflows and ensuring providers have the information they need. • Collaborate with team members via phone, email, and Microsoft Teams to ensure seamless communication and care coordination. • Respond to inquiries and resolve issues with professionalism, maintaining trust and satisfaction for patients and referral partners. • Stay current with system updates and best practices through ongoing training and education. • Perform additional duties and special projects as needed to support our growing team. Qualifications • Licensure: Active, unencumbered LVN license (required). • Education: per state and board of nursing requirements in the state you live in • Experience: Minimum 3 years nursing experience required; 5+ years preferred. • Other Requirements: • Valid U.S. driver’s license. • Ability to pass a criminal background check, MVR check, and non-DOT drug screening. • Proficiency with Microsoft Office Suite, AdvancedMD, and MondayCRM. • Secure and reliable WiFi connection Skills & Abilities • Strong organizational and multitasking skills. • Excellent communication and collaboration abilities. • Compassionate, patient-centered approach to care. • Attention to detail and ability to ensure accurate documentation. • Flexibility to adapt to changing priorities and workflows. • Advanced wound care knowledge a plus Working Conditions • 100% remote with extended periods at a computer. • Light physical activity, including occasional lifting of up to 25 lbs. • Limited exposure to environmental conditions. • Frequent use of computer, phone, and standard office equipment.
