Candidates should have at least 2 years of experience in care coordination and 1 year in a supervisory role within a healthcare setting. A high school diploma is required, while an associate or bachelor’s degree is preferred.

The Supervisor manages a team of Care Anywhere (CAW) Clinical Coordinators and coordinates activities in support of the delivery of Care Anywhere services. In this critical position you will monitor scheduling activities, program referrals, post discharge, ER treat and release follow up, and care coordination activities. The Supervisor will assist the leadership team with mentoring, training and oversight of the performance of assigned employees. Essential to the Supervisor role is contributing to the success of the company. The Supervisor’s actions should be poised toward business growth and comply with Alignment policies. The Supervisor will collaborate with the department Director and upper management team to promote successful growth within the department and throughout the company. This position is responsible for the day to day operational performance of the Care Coordination, CAW team is to ensure timely coordination of services and adherence to regulatory and compliance standards.
