Location: Remote – California (Outside Bay Area) Schedule: Monday–Friday, 8:00 AM – 5:00 PM (Pacific Time) Type: Full-Time-Remote About the Role: Alignment Health is seeking a remote Care Coordinator to support its Special Needs Program (SNP) case management team. This position plays a critical role in assisting members with complex health needs by working closely with the RN Case Manager and interdisciplinary care team. The Care Coordinator helps address care gaps, supports care transitions, and facilitates access to services and resources. Key Responsibilities: Conduct outreach to members to support referrals, authorizations, medication refills, provider appointments, and other care needs. Complete tasks and assessments in the Case Management system for hospital and SNF discharges. Collaborate with RN Case Managers to manage a panel of SNP members. Monitor alerts for changes in condition, admissions, discharges, and diagnoses, and communicate updates. Build trust with members, acting as a liaison and advocate for their care. Document interactions and maintain accurate member records. Support communication with providers, including faxing/mailing of documents and requesting medical records. Attend interdisciplinary team meetings and participate in member case reviews. Maintain confidentiality and meet all departmental deadlines. Required Qualifications: Bilingual in Spanish or Vietnamese (required) Minimum 1 year of experience in a healthcare IPA setting Minimum 1 year of experience with referrals and prior authorizations High school diploma or GED (Bachelor’s degree preferred or 4 additional years of experience in lieu of degree) Basic computer skills (Microsoft Outlook, Word, Excel); typing speed of 25+ WPM Strong verbal and written communication skills Organizational and time management skills with the ability to prioritize in a fast-paced environment Familiarity with Medi-Cal and managed care plans Preferred Qualifications: Medical Assistant or Medical Terminology certification Previous training in medical terminology or clinical support roles Work Environment & Physical Requirements: Remote work setup with standard office equipment usage Must be able to sit, talk, type, and use hands regularly May occasionally lift up to 10 pounds Essential Physical Functions: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
