The Care Coordinator supports members with closing care gaps and addressing care coordination needs as directed by the RN Case Manager. The Special Needs Plan (SNP) team is responsible for the health care management and coordination of care for members with complex and chronic care needs. About the Role: We are currently seeking a Care Coordinator to join our Special Needs Program (SNP) case management team. This fully remote position plays a critical role in supporting members with complex health needs by assisting the RN Case Manager and interdisciplinary care team. You’ll help close care gaps, support care transitions, and ensure members have access to the services and resources they need. Key Responsibilities: Conduct outreach to members for referrals, authorizations, medication refills, and provider appointments Complete tasks and assessments related to hospital and skilled nursing facility (SNF) discharges in the Case Management system Collaborate with RN Case Managers to support a panel of SNP members Monitor member alerts related to condition changes, admissions, discharges, and new diagnoses Build trusted relationships with members as a care liaison and advocate Maintain accurate, up-to-date documentation of all member interactions Communicate with provider offices, including faxing, mailing, and requesting medical records Participate in interdisciplinary team (IDT) meetings and member case reviews Ensure confidentiality of member data and meet all departmental deadlines Required Qualifications: Bilingual in Spanish or Vietnamese (required) Minimum 1 year of experience in a healthcare IPA setting Minimum 1 year of experience handling referrals and prior authorizations High school diploma or GED (Bachelor’s degree preferred, or 4 additional years of experience in lieu of degree) Proficient with Microsoft Outlook, Word, and Excel; minimum typing speed of 25+ WPM Strong verbal and written communication skills Excellent organizational and time management skills Familiarity with Medi-Cal and managed care plans Preferred Qualifications: Medical Assistant certification or training in medical terminology Prior experience in a clinical support or care coordination role Work Environment & Physical Requirements: Fully remote position with use of standard office equipment Must be able to sit for extended periods and regularly use hands to type and handle objects May occasionally lift or move items up to 10 pounds Requires regular talking, hearing, and close visual focus Physical Demands: Regularly required to talk, hear, sit, stand, walk, and use hands and arms Frequently lifts/moves up to 10 pounds Specific vision abilities include close vision and ability to adjust focus Work Environment: This role operates in a remote setting.
