Summary: Advanced Care Partners, one of Inc. 5000's fastest-growing companies, is seeking a kind and dedicated Certified Nurse Assistant/Home Health Aide(HHA) to provide essential care for medically fragile children and adults in their homes or schools. Responsibilities: • Assist our patients with daily care such as grooming, bathing, and toileting. • Assist with medication; take and record vitals. • Assess home situations to determine that our patients are in the best possible environment. • Provide emotional support to our patients and their families. • Provide First Aid and CPR when necessary. • Act as a home health educator for our patients and their families.
