Adfinitas Health has a full time, remote Director of Operations position that will oversee our Physician Group (Maryland Post-Acute Care Facilities) Position Summary The Director of Operations oversees all operational, financial, and quality functions for a physician group serving post-acute care facilities, including skilled nursing facilities (SNFs), long-term acute care hospitals (LTACs), inpatient rehabilitation facilities (IRFs), and assisted living environments. This leader ensures staffing efficiency, provider productivity, financial performance, and adherence to clinical quality standards. The Director of Operations plays a key role in implementing a defined staffing model, launching remote patient monitoring (RPM), and driving a comprehensive quality of care program focused on accurate assessment of severity of illness, pathway-driven care, and reducing avoidable readmissions. Key Responsibilities 1. Staffing Model Oversight • Manage to a defined physician-to-PA/NP visit ratio across all facilities. • Ensure staffing patterns align with census, acuity, facility expectations, and financial targets. • Continuously assess visit distribution, panel sizes, and workload balance to maximize efficiency. • Adjust staffing levels proactively to support operational needs and facility performance. 2. Provider Productivity and Performance • Ensure all providers meet or exceed the organization's minimum productivity standards (e.g., visit volumes, documentation compliance, timeliness). • Develop and maintain dashboards for real-time productivity monitoring. • Implement corrective action or coaching plans when providers fall below expectations. • Collaborate with clinical leadership to integrate performance feedback into annual evaluations. 3. Facility-Level Financial Management • Monitor financial performance and profitability of each facility, including margin, visit profitability, staffing cost, and contract adherence. • Recommend and implement staffing adjustments (up or down) based on financial outcomes and census changes. • Work with finance and revenue cycle teams to correct documentation or coding issues impacting revenue. • Provide routine facility-level performance summaries to senior leadership. 4. Remote Patient Monitoring (RPM) Program Development • Lead the creation and rollout of an RPM program for eligible post-acute patients. • Coordinate the selection and deployment of RPM technology, workflows, and training. • Ensure operational alignment between RPM staff, providers, and partnering facilities. • Monitor compliance, utilization, and financial performance of the RPM program. • Track outcomes such as early decompensation detection and reduced readmissions. 5. Quality of Care Program Implementation • Implement and oversee a comprehensive quality program focused on: • Accurate assessment and documentation of severity of illness at admission • Pathway-driven, evidence-based care across common post-acute conditions • Reducing avoidable readmissions and acute transfers • Develop and track quality metrics including risk-adjusted readmission rates, clinical pathway adherence, and documentation accuracy. • Partner with Medical Directors to drive quality improvement initiatives and provider education. • Ensure compliance with facility, payer, and regulatory quality standards. Additional Operational Responsibilities Oversight and performance monitoring: • Oversee daily operational workflows across multiple post-acute sites. • Ensure documentation is accurate, timely, and compliant with SNF regulatory requirements. • Serve as the operational liaison with facility administrators and hospital partners. • Lead continuous improvement projects related to workflow, efficiency, and provider support. • Support recruitment, onboarding, and retention of providers and operational staff. • Partner with business development to support facility growth, and program sustainability • Monitor vendor performance, compliance, and customer satisfaction: escalate issues, coordinate resolutions and contribute to continuous improvement of vendor relations • Oversee and manage partnerships with Medicare shared savings programs: • monitoring care coordination, adherence to quality measures, ensuring compliance with program activities Qualifications • Bachelor's degree required; Master's degree in Healthcare Administration, Business, or related field preferred. • 5+ years of operational leadership experience in post-acute care, hospitalist programs, or physician services. • Demonstrated experience managing advanced practice clinicians (PAs/NPs) and multi-site staffing models. • Strong understanding of SNF workflows, PDPM considerations, and post-acute regulatory requirements. • Experience with quality improvement, care pathways, and readmission prevention. • Excellent analytical skills with the ability to interpret complex operational and financial data. • Strong leadership, communication, and relationship-building abilities.
