About the position Responsibilities • Conduct preadmission reviews, member assessments, and develop AFC plans of care. • Recruit, train, supervise, and support AFC caregivers, ensuring compliance with training and care standards. • Perform quality assurance tasks, including audits, policy reviews, and report high-profile case issues. • Collaborate with other agencies and maintain professional relationships to promote member care and services. • Maintain certifications, licensure, and compliance with all safety, confidentiality, and regulatory standards. Requirements • Patient care and triage skills. • Medication adherence experience. • Supervisory experience and willingness to travel. • Experience with AFC or similar services. • Familiarity with electronic health records and healthcare systems. • Current RN licensure by the Massachusetts Board of Registration in Nursing. • 2+ years of recent experience in direct care of the elderly or disabled adults. Nice-to-haves • A minimum of one-year supervisory experience preferred. • Experience working in AFC (Home Health) and/or similar settings. • Experience with EMR systems, specifically Icentrix. • Ability to train adult learners. • Basic computer skills and knowledge of remote communication tools like Zoom. • Valid driver's license, private vehicle availability, up-to-date car insurance, and proof of annual vehicle inspection. • Satisfactory completion of CORI, SORI, and National Fingerprint Check.
