Responsibilities: • Deliver direct nursing care in patients’ homes, including assessments, care planning, and interventions. • Monitor and document patient health status, updating care plans as necessary. • Administer medications, wound care, IV therapy, and other prescribed treatments. • Educate patients and families on health conditions, self-care, and treatment plans. • Collaborate with physicians and interdisciplinary team members to ensure high-quality care. • Maintain accurate, timely documentation in compliance with healthcare regulations. • Promote patient safety, comfort, and well-being at all times. Requirements: • Active and valid RN license in the state of Massachusetts • Minimum 1 year of nursing experience (Home Health preferred) • Strong clinical, assessment, and critical thinking skills • Ability to work independently and manage time effectively • Excellent communication and interpersonal skills • Valid driver's license and reliable transportation Ready to Make a Difference?
