Request for Proposal (RFP): Part-Time Evaluation Contractor Issuing Organization: 504HealthNet Date of Issue (Approx.): October 1, 2025 Contract Value (Fee): $15,000 (Total Maximum) Project Context: 504HealthNet is launching an Outreach Program, supported by a Lilly Sponsorship Agreement, focused on providing skin health education and connecting uninsured/underinsured New Orleans hospitality workers to necessary care and resources. This project is part of the Healthy Hospitality initiative. The purpose of this RFP is to secure a qualified, independent contractor to implement the program's evaluation strategy, fulfill reporting requirements, and develop an assessment of the collective care coordination model for hospitality workers. I. Scope of Work (SOW) and Deliverables The Evaluation Contractor will be responsible for a comprehensive set of tasks, categorized into three key phases, from October 2025 through January 2026. Phase 1: Planning and Strategy ● Refine and Implement Evaluation Plan: Review, finalize, and implement the existing draft program evaluation plan, ensuring all metrics align with the goals of the Outreach Program and the specific data requirements of the funder. ● Identify Data Inventory: Critically assess existing data sources and clearly define the necessary new data points required to measure success. ● Link Strategies to Model: Directly link the evaluation metrics, data collection, and qualitative feedback strategies to the design of the 504HealthNet Collective Care Coordination model. Phase 2: Data Management and Collection ● Develop Data Gathering/Collection Strategy: Analyze data collection needs, identify existing data and execute priority initial data collection including managing both quantitative (program metrics) and qualitative (interview/focus group) data streams. Develop plan for additional data collection. ● Data Workflow: Assess, document, and establish a clear, efficient data workflow for the project, ensuring data quality and secure handling. ● Creating Systems to Gather Data: Develop or refine necessary systems and tools (e.g., surveys, tracking sheets) required to efficiently collect the identified data points from community health workers, partners, and participants. Phase 3: Engagement and Reporting ● Facilitate Partner Discussions: Design and facilitate “key informant interviews” with clinical and community partners (e.g., New Orleans Health Department, participating clinics) to gather feedback, validate findings, and assess collaboration effectiveness. ● Facilitate Focus Groups: Design and facilitate focus groups with program participants (workers & employers) to gather in-depth qualitative feedback on satisfaction, barriers to healthcare access & benefits, and preferred outreach & care coordination strategies. ● Develop Impact Statements and Stories: Synthesize complex data into clear, compelling impact statements and articulate qualitative findings into persuasive success stories suitable for funder reporting. This phase culminates in a Final Evaluation Report and a separate Executive Summary/Impact Brief. II. Required Qualifications and Expertise The successful candidate will demonstrate specialized expertise relevant to evaluation planning, community health, care coordination models, and operating efficiently within a constrained grant evaluation environment. ● Relevant Domain Experience: A minimum of five (5) years of demonstrated experience in program evaluation, with direct experience in at least one of the following areas: community health outreach, care coordination/navigation models, or public health evaluation. Experience with the hospitality, tourism & culture industry is valued. ● Budget Management: Proven ability to design and implement a comprehensive evaluation strategy (including all required SOW deliverables) with a total fee constrained to $15,000. ● Methodological Expertise: Advanced proficiency in both quantitative and qualitative research methodologies, with demonstrated ability in focus group design, analysis, and execution. ● Data Translation: Excellent ability to translate technical data findings into clear, accessible, and compelling impact narratives for a non-technical audience (i.e., the funder). ● Education: A Master’s degree or higher in Public Health, Evaluation, Public Policy, Biostatistics, or a related field is highly preferred. III. Timeline and Submission Requirements A. Key Dates | Deadline for Questions | October 7, 2025, 5:00 PM CDT | | Proposal Submission Deadline | October 15, 2025, 5:00 PM CDT | | Anticipated Contract Start Date | October 22, 2025 | | Contract End Date (Approx.) | January 2026 | B. Required Proposal Submission Proposals must be submitted electronically as a single PDF document and include the following three mandatory components: 1. Technical Proposal (3 pages max): ○ Work Plan: Detail your approach to implementing the evaluation, establishing data workflow, running focus groups, and developing impact stories. This plan must adhere to the principles outlined in the attached Evaluation Parameters and Core Questions. ○ Budget: A clear breakdown of the proposed fee (must be between $10,000 - $15,000), including estimated hours and hourly rate. ○ Intent: Confirmation that you accept the contract's scope, budget, and timeline. 2. Curriculum Vitae (CV) or Resume: ○ CV for the lead contractor detailing relevant experience, especially in community health or care coordination evaluation. 3. Work Sample (Optional): ○ One example of a completed evaluation report or impact narrative/storytelling document. 4. List of At least 3 Professional References Evaluation Parameters and Core Questions This document outlines the core questions and minimum data requirements that the external evaluator’s methodology (as proposed in their Technical Proposal) must address. The final evaluation report must directly answer these questions. 1. Program Context and Purpose The evaluation is focused on the Healthy Hospitality Outreach Program, designed to raise skin health and primary care awareness and facilitate care coordination and connection for uninsured/underinsured New Orleans hospitality workers. A draft evaluation plan has been completed. The total remaining budget for all evaluation activities (including the external evaluator’s fee, focus group costs, and survey distribution) is capped at approximately $15,000. The contractor’s fee must be proposed within the RFP’s maximum $15,000 amount. 2. Core Evaluation Questions The final report must address the following key questions: A. Reach and Effectiveness (Quantitative) ● Did the Outreach Program meet its expected activity targets (e.g., number of events, number of workers reached, number of referrals made)? ● What were the short-term outcomes regarding access, specifically the rate at which workers were successfully connected to follow-up services (e.g., primary care or dermatology) within 30 days of referral? B. Care Coordination Model Assessment (Process/Qualitative) ● How effective was the Healthy Hospitality “pop-up clinics” and “workplace canvassing & presentations” in navigating workers from initial outreach to receiving necessary care (including clinical appointments and social needs resources)? ● What lessons learned from these care coordination strategies can be incorporated into the 504HealthNet Collective Care Coordination model? ● What are the primary perceived facilitators and barriers within the New Orleans community for hospitality workers accessing care, based on partner discussions and participant feedback? C. Program Quality and Narrative (Impact) ● What is the overall satisfaction level of program participants (workers and employers) with the education and navigation services provided? ● What is the overall satisfaction level of program partners (504HN staff, member staff and board) with the Healthy Hospitality Outreach Program? ● How can the collective findings be translated into 3-5 clear, compelling impact statements and success stories suitable for funder and public communication? 3. Mandatory Data Areas The contractor must ensure their evaluation design captures data to measure the following areas, as mandated by the Sponsorship Agreement. The technical proposal must demonstrate how these specific data types will be collected and analyzed. Additional metrics for the Healthy Hospitality initiative will be added to the comprehensive evaluation plan. Data Type Example Metric / Focus Area Output Data Number of outreach events, number of educational sessions conducted, total individuals reached with information, total individuals supported with navigation, total individuals screened for skin health. Process Data Defined workflow steps, partner engagement levels, and system-level efficiency of the care coordination strategies. Outcome Data Rates of successful follow-up/connection to care, identification of dermatology-specific services utilized. Qualitative Data Key themes related to patient satisfaction, perceived value of the services, and detailed barriers (logistical, financial, social, etc.) reported by participants in focus groups.
