Role Description The Lead Director, Nursing Operations is an operational leader responsible for advancing clinical excellence, executing nursing workforce strategy, and driving strong patient outcomes across Oak Street Health. This role champions the organization's mission to provide innovative, high quality and safe care to patients by operationalizing strategy, leading teams, and driving results in alignment with organizational goals in value-based care.  The Director partners with medical leaders, operations, and population health teams to optimize care coordination, unified patient communications, and drive KPIs in acute escalations, chronic condition management, and admission avoidance. Success in this role requires excellent leadership capabilities and judgment, a skillful ability to collaborate across the organization, solve complex problems, deliver results, and navigate ambiguity.  Responsibilities Leadership and management • Accountable for operationalizing strategy for nursing and clinical support cohorts to drive organizational performance in quality, population health programs, and patient experience. • Lead multiple teams of different scopes to execute against the above strategic needs for the nursing cohorts, including telephonic triage and direct care delivery in center. • Supports transformation of nursing practice to emphasize evidence-based, proactive and coordinated care delivery models • Executes new clinical care delivery and workforce optimization initiatives • Evolve our central and local nursing leadership structures to keep pace with workforce needs • Define standards of high performance for clinical team roles within the Oak Street care model, measure our performance and partner with leaders to improve where needed. Operational management • In partnership with medical and administrative leaders, structure the job accountabilities of our clinical team members to align with the Oak Street care model to provide the highest impact to patient outcomes, while maximizing team member contribution through “top-of-license†work. • Drive nursing-sensitive performance indicators in chronic condition management, patient education, telephonic triage, and provider support. • Accountable for nursing operations across for 24/7 telephonic nurse triage, in partnership with service operations leaders. • Implement and monitor evidence-based practice standards, clinical protocols, and workflows to improve clinical efficiency and outcomes. • In partnership with population health, drives initiatives to prevent hospitalizations, manage chronic conditions, and drive exceptional patient experience. • Build and sustain efficient and effective staffing models to drive organizational performance. • In partnership with talent acquisition, drive workforce development strategies to recruit and retain a premier nursing workforce. Quality and compliance • Develops and implements evidence-based nursing practice guidelines and protocols, including documentation integrity • Implement best practices in nursing documentation within the Electronic Health Record (EHR) • Other duties as assigned. What we’re looking for Success at Oak Street Health requires a high ability to collaborate, problem solve, manage projects, and be flexible. We’re looking for a motivated clinical leader with: • Proven experience and ability to build, lead and develop diverse teams, engage in multiple perspectives to work toward a common goal • Proven ability to organize, prioritize and manage multiple complex projects while navigating & collaborating with different stakeholder groups • A strategic thinker with strong analytical, problem solving, group facilitation, presentation skills • Ability to exceed in a fast-paced environment while sifting through details quickly and staying focused on goals and deliverables • Outstanding written and verbal communication and conflict resolution skills Minimum Requirements: • Current license to practice as a Registered Nurse • Master’s degree in nursing, Healthcare Administration or related field required; doctorate preferred • 5 or more years of healthcare experience working as an RN, preferably in a primary care office or other outpatient setting • 3-5 years of demonstrated success in a nursing leadership role • Strong ability to navigate change and ambiguity • Excellent ability to partner cross-functionally to drive outcomes • Excellent communication skills and customer service orientation • US work authorization • Someone who embodies being “Oaky†What does being Oaky look like? • Radiating positive energy • Assuming good intentions • Creating an unmatched patient experience • Driving clinical excellence • Taking ownership and delivering results • Being relentlessly determined Why Oak Street Health?
