GENERAL SUMMARY: Comprehensive Community Services (CCS) is a recovery-focused behavioral health program for adults with severe mental illness &/or substance use disorders. CCS is a community-based program in which the majority of services are provided in clients’ homes & communities. The program is person-centered and uses client-directed service plans to describe the individualized services that will support the consumer in achieving their recovery goals. Services are provided by teams of professionals, peer specialists, and natural supports which are coordinated by a CCS Care Coordinator.   Care Coordinators help develop skills for independence, leadership, confidence, and empower individuals to take ownership of their lives and achieve their goals.   ESSENTIAL FUNCTIONS: * Provides care coordination services to each client throughout the duration of enrollment in CCS services by educating consumers/clients on how to utilize community supports, both formal and informal. * Accepts referrals of eligible consumers/clients and initiates the process to assess strengths and needs in a timely manner. * Facilitates team meetings to develop a care plan based on strengths, values, and preferences. * Assists the team in establishing measurable consume/client outcomes and reduce any system barriers that may arise. * Documents consumer/client plans, collects consumer/client outcome data, and prepares all documentation according to program required time frames. * Facilitates annual Functional Screens to ensure consumers/clients meet eligibility requirements. * Carries a caseload ranging between 15-18, based on needs of consumers/clients. * Attends training and ongoing staff development opportunities. * Adheres to St. Charles Youth & Family Services Policies and Procedures. * Other duties as assigned. MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: * Bachelor’s degree required; Master’s degree preferred in a relevant area of education or human services. * 1 year experience working in a mental health related setting. * Knowledge of individual and family issues and dynamics. * Ability to prepare formal reports. * Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors. * Strong verbal and written communication skills. * Valid Wisconsin driver’s license and liability insurance. * Ability to use vehicles for work related duties according to the agency’s insurance liability standards. * Ability to accurately perform record keeping and documentation entries and do so within the required time frame. * Working knowledge of computer applications preferred. WORKING CONDITIONS * General office environment/multiple locations to include community sites and client’s homes. * Travel approximately 70%, including transportation of youth, using Agency’s vehicle(s).  In accordance with the Americans with Disabilities Act, the above is intended to summarize the essential functions of and requirements for the performance of this job.
