SpringVale Health Centers, Inc. is seeking a full-time Licensed Practical Nurse (LPN) or Certified Medical Assistant to provide a variety of nursing duties in a Primary Care and Behavioral Health setting, including medication handling and administration while serving as a liaison between the physician/nurse practitioner and the client for a designated caseload. This position is based in our Dover office. Responsibilities: • Performs nursing related duties as directed by physician or nurse practitioner to assure continuity of care; obtain vitals and complete medication review with clients. • Administers medications to clients; respond to client calls regarding medication questions; give injections according to orders. • Provides a variety of client-related functions including ISP, correspondence, completes BASIS 24, consults with community professionals, agencies and other individuals related to client care, responds to walk-in requests; provides medication education. • Participates in external and internal functions related to performance improvement and monthly staff in-service. • Displays professional attitude conducive to a positive, efficient, and effective work environment. Excellent communication and organizational skills along with the ability to successfully work in a fast-paced environment are essential.
