About the position The Telephone Case Manager RN at Optum plays a crucial role in supporting a diverse member population by providing education, advocacy, and connections to necessary resources. This position focuses on improving health outcomes through effective communication and coordination of care, impacting millions of lives. The role requires various nurse licensure and certification, and involves clinical operations and medical management activities across the continuum of care. Responsibilities • Make outbound calls and receive inbound calls to assess members' current health status , • Make referrals to outside sources , • Identify gaps or barriers in treatment plans , • Provide patient education to assist with self-management , • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction , • Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels , • Work with less structured, more complex issues , • Serve as a resource to others Requirements • Undergraduate degree or equivalent experience , • Current, unrestricted RN license in the state of employment , • 3+ years of experience in a hospital, acute care or direct care setting , • Ability to support a Hybrid work schedule, including consumer centric hours (11a - 8p CST) two days a week , • Ability to type and navigate a Windows based environment Nice-to-haves • BSN , • Certified Case Manager (CCM) , • Case management experience , • Experience or exposure to discharge planning , • Experience in discharge planning or utilization review , • Experience in a telephonic role , • Experience in a leadership, account management or client facing role , • Background in managed care Benefits • $5,000 Sign-on Bonus for External Candidates , • Comprehensive benefits , • Career development opportunities Apply Job!
