Job Summary We are seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to provide exceptional patient care for a new home health agency. Familiar with Wellsky/Kinnser is a PLUS but not required. The ideal candidate will possess extensive clinical experience. The nurse will utilize advanced skills in medical documentation, EMR/EHR systems, medication administration, and patient assessment to ensure high-quality care aligned with regulatory standards such as HIPAA and NCQA. Duties • Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations. • Document all patient interactions accurately within EMR/EHR system Kinnser. • Provide education on health coaching, medication adherence, nutrition, and disease management tailored to individual needs. • Ensure infection control protocols are strictly followed to maintain aseptic techniques and prevent infectious disease transmission. • Maintain compliance with all healthcare regulations including HIPAA privacy laws and NCQA standards. Skills • Extensive clinical experience in diverse specialties: home health, hospital, orthopedics, chronic disease management. • Strong communication skills for effective teaching hospital collaboration and family education; adept at working within multidisciplinary teams to optimize patient outcomes. • Certification in Basic Life Support (BLS) required; additional certifications such as ACLS or PALS are a plus.
