Title: Behavioral Health Care Coordinator Job Summary: Under the direction of the Director of Behavioral Health (DOBH), the Behavioral Health Care Coordinator (BHCC) of Omni Family Health (Omni) provides administrative and professional healthcare support to a growing network of state-of-the-art health centers serving California’s Central Valley. As a member of the organization’s Behavioral Health Department, the BHCC is responsible for monitoring the health status of behavioral health patients and collaborating with healthcare teams to design and implement evidence-based treatment plans tailored to individual needs. Conducts initial assessments, gathers relevant behavioral health information, and identifies key concerns to facilitate coordinated care strategies and ensure high-quality behavioral health management. Job Duties: • Monitors and assesses the condition of behavioral health patients, and prepares written assessments that meet specified standards for Omni patients experiencing mental illness, substance use disorder, or interpersonal relationship challenges. • Conducts initial patient intake with Omni patients and their families, gathering all relevant behavioral health information to accurately identify and address patient needs and guide treatment planning. • Provides psychosocial support to patients, their families, and other vulnerable populations to help them cope with the challenges of chronic, acute, or terminal illness. This support may include emotional counseling, resource navigation, and coordination with healthcare providers to address social and psychological needs. • Cultivates strategic relationships with clinical accounts and stakeholders to enhance service delivery. Partners with clinical teams to develop and execute evidence-based treatments plans. Performs site visits to support program delivery and ensure quality standards. • Supports case management by maintaining regular communication with patients to keep them engaged in their care. • Reviews and monitors adherence to treatment plans, appointments, and referrals, providing necessary follow-ups and making adjustments as needed. • Evaluates and coordinates a wide range of care activities to support patient needs, ensuring that healthcare services and community-based resources are accessible. Facilitates patient access by completing referrals to services such as housing assistance, substance abuse programs, case management, food distribution, parenting classes, and other related resources. • Assists behavioral health providers in completing, or independently complete (within scope of practice), various forms for patients. These may include applications for Supplemental Security Income (SSI), housing for individuals with mental health conditions, In-Home Supportive Services (IHSS), transportation assistance, JV-220 forms, prior authorization for psychotropic medications, and jury duty exemption requests. • Maintains accurate and up-to-date records, and prepares both verbal and written reports and correspondence related to patient care, service coordination, and program activities. Ensures documentation meets organizational standards and support effective communication among care team members. • Assists in planning and executing outreach initiatives to engage patients in wellness and health promotion activities. Enhances patient access to care by building and maintaining relationships with local schools, group homes, and non-profit organizations. These partnerships are leveraged to facilitate timely and appropriate referrals to community-based services. • Ensures the behavioral health department is actively represented in multidisciplinary teams, committees, and meetings, such as those focused on child protection and integrated behavioral health, to support coordinated care and informed decision-making. • Participates in department meetings to share updates, address challenges, and contribute to planning initiatives that support continuous improvement and effective service delivery. • Supports the ongoing development of Omni personnel by delivering in-service presentation to staff, as needed or required, on topics within the scope of the position’s expertise. • Ensures the department provides cohesive support to the mission, vision, and core values while delivering superior customer service. • Works closely with all corporate branches, divisions, and/or departments in accordance with Omni’s policies and procedures. • Other duties and/or responsibilities as the BHCC may be assigned from time to time. • Some travel may be required.
