Job Description Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease. Provides nutritional assessment, education and counseling to individuals and/or groups. Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility. Job Responsibility • Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease. • Assesses nutritional needs and considers patients' cultural, educational, and learning issues. • Provides individualized nutrition education to patients, their families, and/or significant others based on their needs assessment. • Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility. • Communicates patient information to referring providers and appropriate staff in a timely manner. • Documents nutrition information discussed, written materials provided, and evaluation of patient’s receptiveness and comprehension of the education provided. • Performs related duties as required. Job Qualification • Bachelor’s Degree in Food, Nutrition or Dietetics, required. • Registered Dietitian, required. • Certified Dietitian Nutritionist, required • Current CDCES, required OR • Bachelor’s Degree in Nursing, required. • Current license to practice as a Registered Professional Nurse in New York State, required. • Current Diabetes Educator Certification, required. • Minimum of three (3) years progressively responsible, related experience to ensure familiarity with scope of program.
