Hours: 25-30 hours; afternoon and evening hours preferred Position Summary The Women’s Behavioral Health Care Manager plays a key role in LinqCare’s Collaborative Care Model (CoCM)— serving as the bridge between primary care (OB-GYN or women’s health practices), behavioral health, and psychiatry. The Care Manager is responsible for identifying, engaging, and supporting women experiencing depression, anxiety, and other behavioral health conditions across the perinatal and reproductive lifespan. This role combines clinical expertise with compassionate care coordination, ensuring timely access, evidence-based treatment, and measurable improvement in patient outcomes. Key Responsibilities Clinical & Care Coordination Serve as the primary behavioral health contact for patients referred through the OB-GYN or women’s health setting. Conduct initial behavioral health assessments, including PHQ-9, GAD-7, and other validated screening tools. Develop and maintain individualized care plans in collaboration with the primary care provider (PCP), psychiatric consultant, and the patient. Provide brief, evidence-based interventions (e.g., behavioral activation, problem-solving therapy, motivational interviewing). Monitor patient progress through regular follow-up contacts (telephonic, or virtual). Coordinate referrals and transitions of care for higher acuity behavioral health needs. Collaborative Care Model (CoCM) Functions Participate in regular case reviews with the psychiatric consultant and PCP. Track patient outcomes using registry tools and ensure measurement-based treatment-to-target. Document all encounters and updates in the EHR following CoCM billing and documentation standards. Identify barriers to engagement and coordinate with the broader care team to enhance adherence and outcomes. Support ongoing improvement of workflows, protocols, and care pathways for CoCM integration. Education & Communication Provide behavioral health education and support to patients and families, particularly around perinatal mood and anxiety disorders, life transitions, and stress management. Partner with OB-GYN clinical staff to improve recognition and referral of behavioral health needs. Participate in staff meetings, case conferences, and training sessions related to integrated behavioral health. Qualifications Required Master’s Degree in Social Work (MSW) 2+ years of experience in women's health, behavioral health, care management, or integrated primary care. Demonstrated skill in patient engagement, care coordination, and use of measurement tools (PHQ-9, GAD-7). Strong organizational and documentation skills; ability to manage a caseload with multiple priorities. Preferred Licensure: Active state licensure (LMSW/LSW) While this role offers valuable experience, LinqCare does not provide clinical supervision towards independent licensure. Applicants pursuing licensure should plan to secure supervision hours independently. Experience in women’s behavioral health, perinatal mental health, or OB-GYN collaboration. Familiarity with the Collaborative Care Model (CoCM) or population-based behavioral health. Comfort working with telehealth and electronic health record (EHR) platforms.
