Regional Systems Liaison Contractor (January – June 2026) Full Circle Health Network Job Type: Contract (Full-Time Hours) Location: Remote / In the Community (California) Company Overview Full Circle Health Network is an integrated network of nonprofit providers delivering coordinated, community-based services to vulnerable children, individuals, and families across California. Full Circle exists to ensure more Californians can access culturally congruent and trauma-informed care from a high-quality network of community-based organizations that address their whole-person and whole-family needs. We accomplish this primarily through the following core activities: • Serve as a single contracting vehicle for community-based providers to enroll in Medi-Cal managed care plan networks. • Reduce administrative burden for providers so they can focus on serving clients. • Drive improved coordination between providers across multiple systems through technology infrastructure, training, and administrative practice support. Full Circle Health Network is a 501(c)(3) nonprofit and is closely affiliated with the CA Alliance of Child and Family Services, a statewide policy, advocacy, training, and technical assistance organization supporting community-based providers. Contract Position (“Scope of Work”) Job Type: Contract (six months) – January – June 2026 The Regional Systems Liaison will play a pivotal role in building and nurturing relationships with systems throughout the State of California, specifically those related to child welfare and justice-involved sectors. This contract position will work closely with regional systems to educate, increase participation in Medi-Cal programs, and develop key relationships. They will also represent Full Circle, working with public departments, Mental Health, Public Health, Probation, and other social service agencies as needed. Current providers of ECM or CHW services are not eligible for this contractual position. Key Responsibilities Relationship Building • Develop and maintain strong partnerships with social service agencies across the State, particularly those involved in child welfare and the justice system. • Engage with system leaders to understand their needs, challenges, and goals, providing tailored support to increase their engagement and referral of members to CalAIM services. Program Support • Coordinate training and technical assistance for Enhanced Care Management (ECM), Community Health Worker (CHW) Services, and Community Supports (CS). • Collaborate with FCHN’s internal teams to align support services with the regional needs of each system. Advocacy and Outreach • Serve as a liaison between local systems and FCHN, advocating for the needs and perspectives of providers within larger policy and program discussions. • Conduct outreach to identify and engage new systems-based partners, particularly those serving children and families. • Develop a CalAIM toolkit for key regional stakeholders including Child Welfare, Justice, Specialty Mental Health, and Housing departments to support the whole-person, integrated care model. Systems Development • Regularly meet with local CalAIM teams, providers, and other key stakeholders. • Attend outreach events across the State as needed. • Provide education on FCHN and CalAIM services to systems serving children and youth in the assigned region. Grant Management and Reporting • Track and report on the progress of partnership initiatives, ensuring alignment with grant objectives and FCHN’s mission. Qualifications Education • Bachelor’s degree in Social Work, Public Health, Community Development, or a related field. Master’s degree preferred. Experience • Minimum of 3 years of experience working with child welfare, justice-involved, or Behavioral Health program coordination, or in a partnership development role. • Demonstrated experience supporting organizations focused on transitional-aged youth, foster care, or system-involved populations. • Familiarity with Medi-Cal programs, ECM, CHW Services, and Community Supports. Skills • Strong interpersonal and communication skills, with the ability to build and maintain effective partnerships. • Knowledge of the State social services landscape, particularly in the context of child welfare and justice-involved services. • Ability to navigate complex administrative and regulatory environments. • Strong organizational skills with the ability to manage multiple projects and priorities. Additional Requirements • Ability to travel within the region to meet with partners and stakeholders. • Commitment to FCHN’s mission and values, particularly in supporting vulnerable populations and promoting equity.
