Flexible Schedule Options • Full Time: Day shift (3-4 day work week) • Part Time: Day shift (2-3 day work week) • PRN: Day shift What You’ll Do As the Home Health RN, you’ll be the heart of patient care—bringing clinical expertise and compassion directly into the home. You will: • Build meaningful relationships with patients and families. • Create personalized care plans that truly make an impact. • Coordinate care as a trusted Case Manager. • Educate and empower patients and caregivers with confidence. • Collaborate with an interdisciplinary team that shares your commitment to excellence. What We’re Looking For • Current RN license in the State of Arizona. • Valid driver’s license and reliable, insured vehicle. • Strong clinical skills and a passion for patient-centered care. • Ability to work independently and make a real difference.
