Medical Coordinator Job Description Community Living Centers, Inc. is seeking a detail-oriented, conscientious, and dedicated individual to serve as a Medical Coordinator. The Medical Coordinator is responsible for coordinating and monitoring all resident medications, keeping accurate medical records, making all medical appointments, and transporting residents to appointments. This position requires a responsible, caring person, and patient person who loves working with people. Community Living Centers, Inc. provides support to adults that have developmental disabilities in a residential AFC environment. Our goal is to provide safe and comfortable homes that are welcomed in their communities. Full Job Description GENERAL STATEMENT OF RESPONSIBILITIES: Responsible for coordinating all medical and follow-up appointments and completing all required documentation. Provide direct hands-on care and assistance in daily living to individuals living within assigned Residential Group Home. Understand and execute the person-centered plan (PCP) and individual plan of service (IPOS). GENERAL FUNCTIONS: This position description is intended to be a general statement about this job and not to be considered as a detailed assignment. • SERVICE FUNCTIONS: • Maintain close contact with each resident regarding medical needs; is thoroughly knowledgeable of all medical conditions, medications, and maintains contact with all physicians, dentists, psychiatrists, etc. • Attends all medical appointments with residents and completes al required paperwork. If there are two (2) appointments at the same time, the medical coordinator will ask for assistance from the Group Home Manager who will help coordinate the appointments. • Checks in all medication for the group home to ensure that all medications have been delivered for all residents. • Checks all medication sheets against resident's physician prescriptions to ensure that all medication sheets match the prescription. • Be on call 24 hours a day for emergencies and is expected to work evenings and weekend hours as needed or scheduled by Residential Group Home Manager. • Attend Person Centered Planning meetings and provides information regarding the resident's health status. • Communicate with staff and outside agencies regarding the resident's health status. • Provide hands-on care and assistance to individuals we serve including but not limited to mobility/lifting/ transferring, bathing, personal hygiene and grooming, laundry, meal preparation and mealtime assistance. • Assist individuals we serve with activities in daily living including but not limited to household tasks, communication skills, and activities in the community. • Assist in development of individuals' social and recreational interests and skills through appropriate modeling and supportive activities. • Pass medications in accordance with DOCEBO training and physician's orders. • Cooperate with other Group Home staff to assist in meeting the needs of individuals in the Group Home. • Transport individuals and drive agency vehicle as needed, however if agency vehicle is not available it is expected that transportation of the resident to an appointment will occur in staff's personal car. Assure basic cleanliness and care of agency vehicle. • Responding to emergency situations and maintaining timely communication with the Group Home Manager, Directors and co-workers. • Will accurately and efficiently complete all assigned paperwork and maintain all residents' medical files in a timely manner. • Attend and participate in trainings and other professional development activity. • Other duties as assigned. • ADMINISTRATIVE FUNCTIONS: • Maintain current knowledge of Employee Handbook. • Perform all assignments in accordance with established policies and procedures and as instructed by supervisor. • Make regular reports to immediate supervisor concerning assignments. • Participate in cooperative working relationships. • RECIPIENT/STAFF RIGHT FUNCTIONS: • Maintain current knowledge of and act in accordance with relevant recipient right guidelines. • Maintain confidentiality of recipient/staff information. • Complete all Incident Reports by the end of shift and fax them to the appropriate agencies. • Report any recipient rights complaints and problems to the Office of Recipients Rights and then to you supervisor. • HEALTH AND SAFETY FUNCTIONS: • Follow established safety, fire prevention, sanitation and smoking regulations. • Follow universal precautions and prevention and containment of communicable diseases. • Follow and enforce proper lifting procedures. • Use all equipment and supplies in a safe and efficient manner. • QUALITY ASSURANCE FUNCTIONS: • Assist in meeting all established quality assurance standards. • Make recommendations to enhance quality of services provided. Education JOB REQUIREMENTS: • High School Diploma or GED • 2-3 years Supervisory or Medical Coordinator experience in a Group Home setting. • 2 years' experience working in the mental health field. • Completion of all required CMH certifications and training for the counties of Oakland Macomb and Wayne in the State of Michigan. • Able to perform all DSP/DCW duties as needed. • Must maintain a valid and current driver's license. Essential Functions SKILLS: Ability to read and write English and perform basic mathematical calculations. Ability to comprehend and carry out written and verbal instructions. Ability to document accurately and completely. Willingness and ability to work with vulnerable adults. Have a driver's license and a good driving record. Maintain insurance on their personal vehicles. PHYSICAL AND MENTAL REQUIREMENTS: Safely lifts and transfers person served weighing average of 150 pounds on a frequent basis. Pushes person in wheelchair weighing up to 250 pounds on a frequent basis, often for long distances. Frequently stoops, twists, bends, squats and kneels. Lifts overhead. Frequently lifts supplies weighing 25 pounds. Frequently pushes/pulls medication and treatment carts, feels pulse rate and hears and reads blood pressure. Understands and follows instructions. Needs to see and hear persons served, unusual occurrences in living environment of person served. Drives agency van with persons served as passengers; when the agency vehicle is not available the medical coordinator will need to use their own vehicle. Negative TB Test. Satisfactory physical examination. OTHER: Ability to work within the Mission of the agency. Satisfactory verification of criminal/abuse and Sex Offender's Registry checks. Other applicable background checks as required. Reliable ongoing personal transportation.
