We have the perfect job for you! Commonwealth Home Health Network is looking for Registered Nurses to join our team. CHHN provides in-home, skilled and unskilled services to seniors, older adults, and those who are disabled. • Provide skilled care to our patients allowing them to return to independence. • Conduct and complete initial assessment of our patients in order to generate a personalized plan of care in collaboration with physicians and the interdisciplinary team. • Educate our patients and their families on the next steps in continuing care and improvement of day-to-day life. • Coordinate and perform skilled treatments to enhance the well-being of the patient with the goal of enhancing patient health. Qualifications • Valid registered nursing license (required) • 1+ years of registered nursing experience • Associate degree in nursing (required) • Home health experience is a plus.
