In this vital role, you will deliver personalized, compassionate nursing services directly to patients in their homes, promoting health, independence, and quality of life. Your expertise will support diverse patient needs across a broad spectrum of medical conditions, ensuring safe and effective care in a familiar environment. Responsibilities • Conduct comprehensive patient assessments including vital signs, physical examinations, and health histories to develop tailored care plans. • Administer medications via injections, IV infusions, and other routes while adhering to aseptic techniques and safety protocols. • Manage complex medical procedures such as catheterization, wound suturing, ostomy care, and ventilator management with precision and confidence. • Provide specialized care for patients with chronic conditions including dementia, Alzheimer's disease, geriatrics, and long-term disabilities. • Coordinate discharge planning and case management activities to ensure seamless transitions across care settings. • Utilize electronic health record (EHR) systems like Epic or Cerner for accurate documentation of patient care activities and medical records. • Educate patients and their families on disease management, medication administration, nutrition, pain management, and health coaching strategies. Skills • Extensive experience with EMR/EHR systems such as Epic, Axxess, Kinnser • Proficiency in advanced nursing procedures including IV insertion, phlebotomy, laboratory specimen collection/processing, and sterile processing techniques. • Knowledge of ICD-10 coding standards for accurate medical documentation and billing processes. • Strong background in acute care environments such as ICU, emergency room, trauma centers (Level I & II), or hospital medicine. • Skilled in managing complex cases involving infectious disease care, trauma medicine, critical care transport, or post-acute care settings. • Ability to perform detailed patient assessments including physiology knowledge and diagnostic evaluation to inform treatment plans. • Familiarity with specialty areas such as hospice & palliative medicine, behavioral health, pain management, occupational health, and geriatrics. • Experience working with diverse populations including children (pediatrics), neonatal ventilator support, individuals with developmental disabilities or memory care needs. • Competency in performing advanced procedures like tube feeding management, gastrostomy care, spinal taps, or ostomy care while maintaining infection control standards. • Excellent communication skills for triage calls and collaborating with multidisciplinary teams across outpatient clinics or assisted living facilities.
