About 2nd Home Adult Day Care 2nd Home provides comprehensive, culturally sensitive adult day health services that help older adults and adults with chronic medical conditions remain active, engaged, and healthy in their communities. Our programs combine medical monitoring, nursing oversight, therapeutic activities, meals, and transportation in a supportive setting. Position Summary The Community Outreach Coordinator is responsible for developing and executing outreach strategies to increase enrollment and community awareness of 2nd Home’s medical adult day care services in the Perth Amboy region. This individual will build and maintain strong relationships with physicians, hospitals, home care agencies, managed care plans, and community organizations to drive referrals and ensure smooth admissions. Key Responsibilities • Referral Development • Build and manage relationships with hospitals, clinics, physician offices, discharge planners, and community partners. • Conduct outreach visits, in-service presentations, and health-fair events to educate partners on 2nd Home’s services. • Generate qualified referrals and track conversion from inquiry to active enrollment. • Community Engagement • Represent 2nd Home at community meetings, senior fairs, and cultural events. • Develop partnerships with churches, senior housing sites, social service agencies, and advocacy groups. • Coordinate on-site tours and open houses for prospective members and families. • Client Support & Intake Coordination • Work closely with the nursing and intake teams to ensure all required documents and medical clearances are completed. • Follow up with families and referring providers throughout the enrollment process. • Ensure new participants feel supported during their first weeks of attendance. • Marketing & Reporting • Assist in distributing marketing materials across the region. • Maintain accurate outreach logs, referral databases, and weekly activity reports. • Collaborate with leadership to set and meet monthly enrollment targets. Qualifications • 2+ years of experience in community outreach, healthcare marketing, or case management (adult day care, home care, MLTC, or long-term care experience preferred). • Strong interpersonal and communication skills with the ability to engage seniors, families, and healthcare professionals. • Bilingual in English and Spanish preferred. • Valid driver’s license and reliable transportation required. Performance Expectations • Achieve monthly client acquisition targets as defined by the Center Director. • Maintain accurate referral records and meet outreach visit goals. • Exemplify professionalism and empathy when representing 2nd Home in the community.
